Appointment Requests

Request an Initial Consultation

Please fill out the form at the bottom of the page to request an initial consultation with Davis Advocates.  Please understand that the appointment is pending until you have received a confirmation e-mail from Samantha Davis.  Please do not schedule an appointment an hour before or an hour after a blocked off time in order to allow for travel time.  There may be times that show available but Samantha may not be due to extended travel time.  If that is the case, she will e-mail you with alternate dates and times.

You will be contacted after your appointment time has been confirmed to discuss the location for the meeting.  Mrs. Davis likes to meet with clients at their home, a local library, a Starbucks or other restaurant.  Feel free to indicate on the form if you have a preference.

We understand that life happens and you may need to reschedule your appointment.  Please kindly call or e-mail our office if you need to reschedule your initial consultation.  Davis Advocates will reschedule a free initial consultation one time.  If an appointment has to be rescheduled a second time then Mrs. Davis’ standard hourly rate of $75/hour will apply and one hour’s time will need to be paid in advance of the appointment.  Appointments will not be rescheduled a third time.
